Health Records Requests
Freedom on Information
Under the Freedom of Information Act 1982, the public can access their health records.
Charges associated with requests include:
- $29.60 application fee
- 20 cents per page photocopying/printing charge
- $10 postage charge, if applicable
- Other charges may apply
The application fee is waived for applicants who can provide a photocopy of their certified Social Security card (i.e. pension or healthcare card).

Time of birth
If you are seeking a birth date and time, this can be provided without going through Freedom of information, on a typed Time of Birth letter. Please contact our FOI Office on 4215 1168 or email [email protected] and this can be arranged.
Photo proof of ID will be required and a charge of $10 applicable. Fee will be waived for people who hold a healthcare or pension card.
My Health Record
Barwon Health is linked with the My Health Record and electronically sends patients' discharge summaries and medications information to those patients who have a registered My Health Record. From 1 February 2019, a My Health Record will be created for every eligible Australian who has chosen not to opt-out by the deadline of 31 January 2019.
Patients can visit www.myhealthrecord.gov.au to find out more. Patients who need extra support can call the Help line on 1800 723 471.
