Palliative Care: Referrals

Referrals to the community program, Palliative Care Unit, and outpatient clinics including cachexia, psychology and accredited mental health social work services, grief and bereavement support from general practitioners and medical specialists in the Barwon south west region can be made via phone (03) 4215 5700 or fax (03) 4215 6390.

People requiring Community Palliative Care are generally referred by their general practitioner or medical specialist, however we do accept referrals from patients, family, and from health professionals and community agencies. We are happy to answer questions from anyone about referrals to palliative care

For Health Professionals

Referral templates

Referrals to the Acute Consultancy Team at the Geelong Hospital are to be made from the patients treating team using the Bossnet e–referral system.

Referrals for volunteer support can be made via email, telephone or fax. Please include the following information:

  • UR number
  • Patient name
  • Support type e.g. respite, companionship, transport
  • Phase/complexity

Please ensure patients who are referred for volunteer support do not require assistance with feeding, medication, toileting or other nursing type care – our volunteers are unable to assist with these tasks.

Please also ensure transport requests are submitted with as much notice as possible.
