Paediatric Home and Community Care (P-HACC)

P-HACC is an innovative and expanding service established by the paediatric and neonatal unit management teams. It is operated by a team of experienced midwives from University Hospital Geelong's Special Care Nursery in cooperation with paediatricians. These nurses provide care and support for parents with babies that have required specialised care following birth.

Our goals:

  • To provide safe and effective home and community care
  • To promote continuity of care between hospital and the community
  • To provide families with choices and options of taking their babies home earlier
  • To provide family focussed care
  • To compliment existing nursing services, for example maternal and child health nurses.

Services we offer:

  • Daily visits for babies under 2kg. progressing to twice weekly visits as required. Babies will be weighed, temperature taken if necessary and support and guidance given to the parents as required.
  • Provide up-to-date information on baby care
  • Liaison with and referral to other community nursing and support services.
  • Liaison with paediatrician in regards to ongoing care.

Who is eligible?

Babies that have been admitted to the Special Care Nursery and have further care needs are eligible.

These babies may have been admitted for a variety of reasons, such as:

  • Prematurity
  • Intra-uterine growth restriction
  • Babies of diabetic mothers
  • Chemical dependence
  • Oxygen therapy
  • Special needs, social reasons.
