Connected Care is a team of experienced health workers who see people in the community; we hope the following information will assist you in making a decision about whether our service will be of use to you.
How can Connected Care help?
The Connected Care service work with you to determine your support needs. We aim to improve your ability to self-manage your conditions and circumstances.
Areas we often help with are:
- Assistance and education to manage your health
- Discuss your needs with people you identify as appropriate for example:
- You and your family or carer
- Other health providers including your GP and hospital
- Other support services
Link you with appropriate services
- Coordinate services and appointments
- Facilitate communication between multiple agencies and professionals
- We aim to ensure you are receiving the right care, in the right place, at the right time.
Who can get help from Connected Care?
If you are living with a chronic disease and have complex needs, you may be eligible for Complex Care Service.
We help people who are living with:
- Chronic illness
- Issues with wellbeing, confidence, self-esteem, social, spiritual and emotional health
- Complex care needs
Where do I go for Connected Care?
We come to you at home or near to you in your local community
