The Barwon Health Social Supports Program (SSP) is located in Belmont. The purpose of the program is to engage people in meaningful social activities that draw on personal strengths, encourage independence and involve them in their community.
Our program supports people:
- with no or few social supports
- looking for more social interaction
- living alone
- the elderly
- with disabilities
- needing respite for their carers
The Social Supports Program was awarded the 2024 Barwon Best Care – Connected Care Award.
Helping you get out and about in the community
We offer a warm environment, caring staff and an opportunity to make new friends. The program works with you and your family/carer or support services in tailoring activities that meet your needs.

Referrals
This program is funded by the Commonwealth Home Support Program (CHSP) and clients over 65 years of age need to call My Aged Care on 1800 200 422 to request an assessment and a referral code for Social Supports Group. Then please call our Intake Worker on 4215 7049 to arrange a free trial visit.
This program is also funded by the Home and Community Care Program for Younger People (HACC PYP) and clients under 65 years of age just need to call our Intake Worker on 4215 7049 to arrange a free trial visit.
Cost
A low-cost, subsidised program fee. Extra community activities are available to suit every budget.
Transport
Transport is available for clients if needed.
Meals
Morning tea and a three-course lunch are provided each day.
