How do I access my health records?

If you want to know more about your care, you can:

  • Ask a member of the clinical team treating you
  • Get a second opinion if you like
  • Choose another person or organisation to speak on your behalf
  • Ask about the qualifications and experience of staff providing your care and treatment

Freedom on Information

Under the Freedom of Information Act 1982, the public can access their health records.

Charges associated with requests include:

The application fee is waived for applicants who can provide a photocopy of their certified Social Security card (i.e. pension or healthcare card).

Download the FOI application form or contact the FOI Office on 4215 1168 or [email protected] to arrange to collect or be mailed the form, or to discuss an application.

  • $30.60 application fee
  • 20 cents per page photocopying/printing charge
  • $15 postage charge, if applicable
  • Other charges may apply

Time of birth

If you are seeking a birth date and time, this can be provided without going through Freedom of information, on a typed Time of Birth letter. Please contact our FOI Office on 4215 1168 or email [email protected] and this can be arranged.

Photo proof of ID will be required and a charge of $10 applicable. Fee will be waived for people who hold a healthcare or pension card.


My Health Record

Barwon Health is linked with the My Health Record and electronically sends patients’ discharge summaries, pathology results and medications information to those patients who have a registered My Health Record. From 1 February 2019, a My Health Record will be created for every eligible Australian who has chosen not to opt-out by the deadline of 31 January 2019.

Patients can visit www.myhealthrecord.gov.au to find out more. Patients who need extra support can call the Help line on 1800 723 471.
