Barwon Health has received $426,131 in grants to improve innovative services and infrastructure for end-of-life care in patients’ homes.
A Palliative Care Service Innovation Grant of $151,865 will support a new Palliative Care at Home model, enabling extended home care for patients who may otherwise need to be cared for in hospital.
An additional infrastructure grant of $274,266 will cover improved clinical equipment and a car fleet extension to better support clients and carers delivering palliative services in the community.
Palliative Care Program manager Jacqui White said the funding would make a huge difference to the community services offered to patients needing end-of-life care.
“This will broaden our service and allow us to upgrade our fleet vehicles to continue caring for people in their homes, while upgrading the clinical equipment we use for that care,” she said.
“We’re pleased that we can now advance some of the innovations we’ve been planning to improve the provision of terminal care in patients’ place of choice, which is often at home.”
The State Government grants aim to develop and test new models of care, improve coordination of care, and support better integration between service providers.
