Advance Care Planning

Who would make medical treatment decisions for you if you were too sick to do this for yourself? Would they know how to make decisions or what you would want?

Advance care planning is a process of planning for future health and personal care, whereby the person’s values and preferences are made known so that they can guide medical decision-making at a future time when the person cannot make or communicate their decisions.

Advance Care Planning:
Assists individuals to discuss and document their values and preferences about their future medical treatment, and complete an Advance Care Directive and appoint a Medical Treatment Decision Maker if they wish to. The process promotes an individual's understanding about their future treatment options and is supported by current legislation, the Medical Treatment Planning and Decisions Act 2016.
Understanding and respecting your values will help your doctors and your family to make better choices about your medical treatment. With this information, they can feel reassured that they are respecting your wishes.

Our services

Barwon Health’s Advance Care Planning program provides a service to Barwon Health consumers and community. This includes a consultation service to assist consumers to discuss and develop an Advance Care Directive, including appointment of Medical Treatment Decision Maker and promotion of community awareness by providing a guest speaker for groups or organisations upon request.

How to prepare for your appointment

MyValues
MyValues is an online survey which may help you understand and explain your values in relation to life, death and medical intervention.

Understanding and respecting your values will help your doctors and your family to make better choices about your medical treatment. With this information, they can feel reassured that they are respecting your wishes.

On completion of the survey, a concise report is generated which you can print or insert into the Victorian Advance Care Directive for Adults document.

What to bring

If available, please provide the following at your first appointment:

• Your MyValues survey report, if completed or any existing Advance Care Directive document.
• Any prior Appointment of Medical Treatment Decision Maker including Enduring Power of Attorney – Medical Treatment
• Contact details, including full name, DOB, address and phone no.s of the people you would like to appoint as Medical Treatment Decision Makers.
• Family member(s) or support person.
