Service Description:

The Community Rehabilitation Centre provides team rehabilitation on an outpatient basis to people living in the community. Clients are provided with flexible, goal orientated, individually tailored programs, following team assessment.

Referral Guidelines:Referral from a health professional or agency is required. All referrals are triaged, and if not suitable for the Community Rehabilitation Centre, will be forwarded to an appropriate agency.
