What is the Home Based Rehabilitation Program? (HBRP)
The HBRP offers individual rehabilitation programs in the client's home environment. The aim is to enhance the individual's potential to return to previous activities.
The HBRP offers services to people who are experiencing difficulties following a new health problem (eg: stroke, joint replacement) or an existing condition (eg: Multiple Sclerosis, Parkinson’s Disease).
In addition, the HBRP provides allied health services for other Barwon Health initiatives such as Hospital in the Home and the Geriatric Evaluation and Management Package.
What is provided by HBRP?
The HBRP offers a personalised multi-disciplinary rehabilitation program that could involve the following team members:
- Medical Rehabilitation Specialist
- Occupational Therapist
- Physiotherapist
- Speech Pathologist
- Social Worker
- Allied Health Assistant
- Program Coordinator
Equipment and Costs
The HBRP has equipment for loan whilst the client is receiving rehabilitation through the program. HBRP therapists will assist with equipment provision and purchase for long-term needs.
There will be a charge of $7.00 per day (max. $21.00 per week) associated with the provision of this program. Clients will be sent monthly accounts.
Admission Criteria
Clients will need to:
- Be able to benefit from a rehabilitation program at home
- Be medically stable
- Have a safe, suitable home environment
- Have access to a carer or support person who is willing and able to assist when necessary
- Have a General Practitioner who supports admission to the program
- Reside within City of Greater Geelong, Borough of Queenscliff or Surf Coast Shire Council boundaries
Rehabilitation Program
An integrated rehabilitation program will be developed with the client, family, carer/support person and staff, aimed at achievement of the client's functional goals.
While on the program each client remains under the care of his or her general practitioner. Each client’s rehabilitation program is reviewed regularly in accordance with the client’s progress. On discharge, referrals may be made to other rehabilitation services or support services in the community when appropriate. The program is available Monday to Friday.
