Service Description:

What service is provided?

Rehabilitation programs are provided for people with orthopaedic, amputee, neurological, trauma and acquired brain injury conditions. There is also a convalescence service for older people with complex and/or chronic conditions requiring rehabilitation following an acute hospital stay.

Rehabilitation assists the natural recovery process and sometimes involves learning new ways of doing things.

Rehabilitation involves health care professionals working with an individual to overcome a physical disability that limits that person’s capabilities and ability to operate within their environment.

Barwon Health recognises that the key to successful rehabilitation is to maximise the individual’s ability to participate in key activities that are integral to the individual’s lifestyle. In addition to this, it is imperative to remove the disabling barriers of the environment that limit the patient’s participation both within the inpatient environment and the patient’s own living environment.

The rehabilitation team will work with you and your family, to develop the best possible program to help you reach your goals, return to participating in your daily tasks and interests, to help you feel well again and to stay healthy.

The Speech Pathology service:

 

· Provides assessment, diagnosis and management of the full range of communication and swallowing disorders of neurological origin resulting from traumatic brain injury, stroke, other acquired brain injury, or ageing.

 

· Implements appropriate clinical management procedures based on world’s best practice, diagnostic and prognostic indicators and technical knowledge.

 

· Provides a combined, client centred, and coordinated inter-disciplinary service within specialist teams to assist patients in achieving their optimum level of independence.
