Service Description:The Clozapine Program is operated by Barwon Health mental health nurses who support people with severe mental illness who are prescribed Clozapine to remain in primary care, under the care of their GP.

Services include providing Clozapine training and support to the GP, assisting the coordination of pathology testing, facilitating six monthly psychiatrist reviews, monitoring the client’s progress and coordinating services. Referral is via specialist mental health services.
Referral Guidelines:Via specialist mental health services only
Eligibility Criteria:A person, who has been prescribed Clozapine, is under the care of a GP who has received appropriate Clozapine training and who has agreed to provide ongoing care.
