Service Description:

Mental health nurses collaborate with participating GPs within the general practice to assist people who have a severe mental illness during periods of significant disability.

Services include assessment and ongoing review, liaising with family and carers, monitoring and assisting with medication adherence, providing information on physical health care, contributing to care planning, coordinating services and improving links to other service providers.

Referral Guidelines:GP referral only. Fax the referral to 5222 4798. Appointments are made within the general practice.
Eligibility Criteria:Ages: 16 years and over
