Service Description:

The Barwon Health Palliative Care Program provides care to patients and their families within the Barwon South West Region.

Our vision is to be the leading provider of Palliative Care providing cohesive, proactive and interdisciplinary service, focusing on needs-base care.

The services we offer include:
- Palliative Care Consultancy Service at the Geelong Hospital: available to all acute areas of Barwon Health. 
- Community Palliative Care: providing specialist palliative care in the home. 
- Palliative Care Unit at the McKellar Centre: a 15-bed facility providing high quality patient and family orientated care, including pain and symptom management, bereavement support to family and friends. 

Outpatient Clinics at the Geelong Hospital and McKellar Centre: 

  • Cachexia Clinic: characterised by reduced appetite and weight loss including loss of muscle mass. The clinic provides medical, dietician and physiotherapy assessments to assist patients and carers with the management and treatment options.       
  • Psychology Clinic: to assist people in developing skills to improve relationships and reduce the impact of stress, pain and mental health difficulties, such as depression and anxiety.
  • Palliative Care Clinic: provides comprehensive assessment and management of complex symptoms, and end of life care planning, for patients with advanced life-limiting illness.

GPs and Clinicians please refer patients by letter, addressing the appropriate clinic and faxing referral to the number listed above.

After hours access for Community Palliative Care Program patients
Patients who have been admitted to the Community Palliative Care Program and GPs calling our service after-hours will be automatically diverted from our central number to the Geelong Hospital switchboard, to be connected with the after hours service from 4.30pm to 8.30am, Monday to Friday, weekends and public holidays.

Telephone advice will be provided to our current clients by Caritas Christi Hospice Palliative Care Nurses and/or our Medical Staff by calling the same telephone number.

Central Referral Point for Palliative Care
The centralised referral point will assist you in accessing the following services offered by Barwon Health by phoning (03) 5279 2800, or fax (03) 5279 2801 (during business hours).

Referrals to the Community Program, Palliative Care Unit, and Outpatient clinics including Cachexia & Psychology services, Grief & Bereavement support by contacting the numbers listed above.
