Central Referral Point for Palliative Care
Barwon Health is pleased to announce that a central referral point for the Palliative Care Service has now been created. This centralised referral point will assist you in accessing the following services offered by Barwon Health:
- Palliative Care Consultancy Service at Geelong Hospital – available to all acute areas of Barwon Health.
- Community Palliative Care – providing specialist palliative care in the home.
- Palliative Care Unit at the McKellar Centre – a 15-bed facility providing high quality patient and family orientated care, including pain and symptom management, bereavement support to family and friends.
- Outpatient Clinics at Geelong Hospital and McKellar Centre.
Referrals can be made by General Practitioners in the Barwon South Western Region from 8:30am to 4:30pm Monday to Friday by contacting ph. 5279 2800, or fax 5279 2801.
In addition to the office hours referral service, after hours access to telephone advice provided by Caritas Christi Hospice Palliative Care nurses will be available on the same telephone number. Patients who have been admitted to the Community Palliative Care program and GPs calling our service will be automatically diverted from our central number to the Geelong Hospital switchboard and connected with the after hours service from 4.30pm to 8.30am, Monday to Friday and on weekends and public holidays.
