Hospital Admission Risk Program



WHAT IS HARP?


The Hospital Admission Risk Program (HARP) provides a specialised service that aims to support and strengthen the role of clients in managing their health problems. Barwon Health is committed to providing effective health care to clients with chronic health conditions through this program.



The Program is designed to assist clients living with a chronic disease to improve their health outcomes and help people learn to cope with their life-long symptoms. HARP is centered on respecting the client’s values and preferences. HARP aims to improve services and communication between the hospital and community health sector.



WHAT SERVICES DOES HARP PROVIDE?


Barwon Health’s Hospital Admission Risk Program (HARP) provides community based support services to those with chronic health conditions who live in the Geelong and Surf Coast Community such as:



  • One-to-one support to clients and their families or carers

  • Linkages to GP’s and other support services

  • Exercise and support groups for those with respiratory conditions

  • Community based client and carer education and support groups for those with Chronic Heart Failure

  • Chronic Heart Failure Clinics

  • Diabetes Management

  • Smoking cessation

  • Self Help Anxiety & Depression Education & Support (SHADES) Groups
