Service Description:


The Home Referral Service provides home-based health care and services:



  • To assist patients to recover following their discharge from hospital

  • To ensure patients are linked to longer term community support services as necessary

  • To avoid hospital admissions by providing acute care at home

  • To assist with safe and timely discharge

The service includes the Hospital in the Home and Post Acute Care programs.


All patients receiving care and support through these programs will need to be assessed as suitable for the program and meet the Home Referral Service eligibility criteria.


All patients receiving home based care and services through Home Referral Service will:



  • consent to participate in the program

  • have their individual needs assessed and a care plan developed

  • have their care and recuperation monitored by staff from Home Referral Service

  • have their care needs evaluated for ongoing support as required
