The Community Health Division provides both general and specialised health services to individuals and groups in their homes, schools and workplaces, at local health centres and other community locations. Services are organised wherever possible in multi-disciplinary teams and provide treatment, advice, support and care to individuals and families, and health promotion and information to groups and communities.
Community Health Division Services include:
- Primary Care (Community Health Nursing, Physiotherapy, Podiatry, Occupational Therapy, Psychology/Counselling/Social Work, Dietetics, Health Education, and Speech Therapy)
- Day Activity Centres (Aged and Dementia)
- Women’s Health
· Family Planning
· Youth & Homeless Youth Counselling & Health Support
· Community Dental (including School Dental Services)
- Domiciliary Care (District Nursing, Domiciliary Midwifery, Home Palliative Care)
- Carer Respite
- Regional Palliative Care
- Paediatric & Adolescent Support Service
- Quality and Education
- Immunisation
- Home Referral Service (including Hospital in the Home and Post Acute Care)
- Hospital Admission Risk Program (Complex & Chronic Care Team and Integrated Disease management)
The Division also has Auspice/Lead Agency responsibility for the operations of the following Projects and Services:
