Service Description:District Nursing no longer focuses solely on the physical care of the person, but cares for the whole person in their total environment as member of a family and a community. This care takes into account their social emotional and spiritual needs and includes support of carers and family. Our services are provided to:
  • Older persons who are frail or who have a chronic or degenerative illness and need nursing care to remain in the community.
  • People of any age who need care immediately following discharge from hospital/day surgery.
  • People of any age needing palliative care.
  • People of any age who have a disability and who need nursing assistance to manage their health care needs.

Our nurses will work in partnership with you and your carer to identify and achieve the best possible health outcomes.
