| Service Description: | District Nursing no longer focuses solely on the physical care of the person, but cares for the whole p[erson in their total environment as members of a family and a community. This care takes into account their socal, emotional and spiritual needs and incudes support for carers and families.
Our services are provided to:
- Older persons who are frail or who have a chronic or degenerative illness and need nursing care to remain in the community.
- People of any age who need care following discharge from hospital/day surgery.
- People of any age needing palliative care.
- People of any age who have a disability and who need nursing assistance to manage their health care needs.
Our nurses will work in partnership with you and your carer to identify and achieve the best possible health outcomes.
