Service Description:

A stomal therapy nurse is available for the Urology clinic

 

Urology Specialist Nurse (USN):

The USN is a member of the Urology team and is available for information and feedback to GPs.  The USN attends all the clinics and is also available everyday to facilitate management of patients who develop problems post operatively or between appointments

Referral Process:

There are five (5) different referral templates for urology referrals.  Urology General, Raised PSA, Haematuria, Enlarged Prostate, Renal Colic.

All templates (excl. urology general) contain embedded referral guidelines giving access to recommendations for investigations and conservative therapy that may be useful before referring the patient

• Complete appropriate urology referral template (see links below to download)

• All referrals must be faxed to outpatients

• All referrals are triaged for urgency by the Urology Specialist Nurse

• Provision of appropriate clinical information within the "reason for referral" section of the referral form is essential for accurate triaging and access to services.  This will also assist with arranging appropriate Allied Health assessments where necessary

• Both GP & patient will be notified of receipt of referral and expected waiting time for appointment

• Both GP & patient will be notified if a referral has been rejected

Word Surgical Referral  

Reassessment Process:

For patients who require reassessment regarding planned surgery or planned appointments, a new referral should be faxed through.  In order to facilitate access the new referral must contain the following information:

• State the referral is an update for a current patient

• Date of original referral

• Reason for requesting a review of referral.  Eg. deterioration in clinical condition, pathology

The GP will be notified of the outcome of the re-triaging process
