Advance Care Planning

Respecting Patient Choices Program

 

Respecting Patient Choices is an advance care planning program which:

  • Promotes an individual's understanding about their future treatment options
  • Assists individuals to document their wishes and preferences about their future medical treatment in an Advance Care Plan

In Victoria the program relies on powers enabled by the Victorian Medical Treatment Act (1988).  The Act enables a person to appoint someone who can act as his or her Medical Enduring Power of Attorney (MEPOA) for medical treatment.

 

When you or one of your staff identifies a patient whom you feel may benefit from referral to the Respecting Patient Choices Program for advance care planning (or are approached by a patient who requests referral) please provide the details by:

Completing an Advance Care Planning Program Referral Form.  Print and fax to 03 5221 7986

 

 OR

 

Phone the RPC office on 03 5226 7006

Office hours Mon-Fri 0900-1230

 

Arrangements will be made for an RPC consultant to contact the patient at the earliest opportunity.
