Western Victoria Primary Health Network

As one of the 31 Primary Health Networks (PHNs) set up across Australia, this community organisation is responsible for increasing the efficiency, effectiveness and coordination of healthcare services for patients, particularly those at risk of poor health outcomes. Working closely with the community and health industry partners, Western Victoria PHN is committed to strengthening the primary healthcare system and operates from four regional centres in Horsham, Ballarat, Geelong, and Warrnambool.
