PROJECT NUMBER
All projects are identified with a unique number that remains for the life of the project. Please organise this number as soon as practicable so we can log you onto our systems.
ORGANISATIONAL REPRESENTATIVE
If you are an applicant who is not a staff member of Barwon Health, you must include at least one senior member of staff on the research team whose role is to be the ‘organisational sponsor / contact’ of the project.
This person should:
• Have a close working relationship with the project
• Act as a resource and provide oversight on behalf of Barwon Health
• Obtain permission and sign-off from relevant departments
• If required, organisation of on site access which will require:
þ Privacy, Confidentiality and Security Agreement
þ Current Police Check
þ Curriculum vitae
þ Barwon Health Identification badge with relevant access
